Job Description
We are seeking a skilled Home Health Care Coordinator to join our team at Apria. This role involves coordinating home health care services for patients, requiring strong communication and organizational skills.

About the Role
As a Home Health Care Coordinator, you will be responsible for coordinating home health care services, including delivering and setting up durable medical equipment, instructing patients on basic use of these devices, and responding to emergency calls during regular business hours and on an on-call basis. You will also complete necessary paperwork, including daily delivery logs and vehicle maintenance logs, and maintain supply components of patient profiles.

Requirements
To succeed in this role, you should have at least one year of related experience and a high school diploma or equivalent. A valid driver's license and successful completion of Apria Healthcare's Driver Training Program are also required. You must be able to lift items ranging from 10 to 160 pounds and push or pull objects weighing up to 60 pounds.
