About the Role

We are seeking a skilled and compassionate Physical Therapist to join our team at Parx Home Health Care. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients.

Job Summary

The successful candidate will be responsible for providing high-quality physical therapy services to patients in their homes and communities. This will involve conducting comprehensive evaluations, creating personalized treatment plans, and educating patients and their families on proper exercise techniques and injury prevention methods.

Responsibilities

  • Conduct thorough physical therapy assessments to identify patient needs and develop effective treatment plans.
  • Provide one-on-one physical therapy sessions to patients in their homes, focusing on improving mobility, strength, and function.
  • Develop and implement customized exercise programs to promote patient recovery and prevent future injuries.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure seamless care coordination.
  • Maintain accurate and detailed records of patient interactions and treatment plans, adhering to state and national regulations.

Requirements

To be considered for this role, you must possess:

  • A valid Physical Therapy license in the state of Florida.
  • A minimum of one year of experience in physical therapy, preferably in a home health care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • A passion for delivering high-quality patient care and making a positive impact on the lives of others.
