We are seeking a compassionate and caring individual to join our team as a Hospice Support Team Member. This role involves working with patients who require personalized care and support. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

**About Our Patients:**
Our patients come from all walks of life, and each one has their own unique story and challenges. As a Hospice Support Team Member, you will work closely with patients and their families to understand their needs and preferences, and provide care and support that is tailored to their individual needs.

**Responsibilities:**
- Providing emotional support and companionship to patients and their families.
- Assisting with daily activities such as bathing, dressing, and grooming.
- Preparing and serving nutritious meals and snacks.
- Managing medications and treatments as prescribed by healthcare providers.
- Monitoring patients' conditions and reporting any changes to the Case Manager.

**Requirements:**
- At least 16 years of age.
- Certified by the state as a nurse aide.
- Maintains a current CPR Certification.
- List on the state aide registry with a clear record able to deliver care to patients.
- Understands hospice philosophy, comfortable providing specialized care to the terminally ill.

Work Schedule:
- Monday-Friday, 8am-5pm.
