Nurse Practitioner

SSM Health is seeking a skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our medical staff to ensure the best possible outcomes for our patients.
Responsibilities- Provide Direct Patient Care: Implement and monitor patient care plans, monitor and record patient condition, and communicate with patients and families to ensure their needs are met.
- Coordinate Patient Care: Serve as a primary coordinator of all disciplines for well-coordinated patient care, including assessing and coordinating patient discharge planning needs.
- Communicate with Healthcare Team: Note and carry out physician and nursing orders, and work collaboratively with other healthcare professionals to ensure seamless patient care.
- Apply Evidence-Based Practice: Apply the existing body of evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs.
- Meet Professional Standards: Demonstrate the professional nursing standards defined in the professional practice model, and use the ANA Code of Ethics for Nurses to guide response to current and evolving healthcare and nursing needs.
- Education: Graduate of accredited school of nursing or education equivalency for licensing.
- Experience: No experience required.
- Licensure: Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA), and Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), or equivalent licensure in other states.
