- Monitor, record, and communicate patient condition as appropriate to ensure seamless care coordination.
- Serve as a primary coordinator of all disciplines to ensure well-coordinated patient care.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
- Apply the existing body of evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs.
- Graduate of an accredited school of nursing.
