We are seeking a dedicated and compassionate Nursing Care Coordinator to join our team at Ochsner Health. In this role, you will serve as a liaison between patients, caregivers, healthcare providers, and multidisciplinary team members.
Job Summary:This position is responsible for facilitating movement along the healthcare continuum to ensure quality, cost-effective outcomes are achieved in collaboration with the multi-disciplinary team. The successful candidate will work closely with physicians to coordinate patient care plans and communicate effectively with all members of the healthcare team as a patient advocate.
Responsibilities- Provides continuity of care by ensuring smooth transitions between care settings
- Develops relationships with patients and their multidisciplinary teams to facilitate and navigate subsequent treatment and follow-up
- Manages high-risk, complex patient care with the goal of minimizing readmission
- Communicates with all members of the healthcare team as a patient advocate
To be considered for this role, candidates must have:
- A minimum of 2 years of nursing experience
- Associate's or bachelor's degree in nursing
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills
- Ability to work a flexible schedule, including weekends, holidays, and on-call availability
Ochsner Health offers a competitive salary range of $85,000 - $110,000 per year, depending on experience. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
