Job Summary

CenterWell Senior Primary Care is seeking a highly skilled and compassionate Field-based Nurse Practitioner to join our team. As a key member of our primary care team, you will provide high-quality, patient-centered care to seniors in their homes, focusing on proactive, preventive care and addressing the physical, emotional, and social well-being of our patients.

Key Responsibilities
  • Provide comprehensive primary care services to seniors in their homes, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
  • Develop and implement personalized care plans that address the unique needs and goals of each patient.
  • Collaborate with our interdisciplinary care teams to ensure seamless coordination of care and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices in primary care, and apply this knowledge to improve patient care and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of new programs and services that enhance patient care and satisfaction.
Requirements
  • Master's degree in Nursing or related field.
  • Minimum 2 years of experience as a Nurse Practitioner in primary care or a related field.
  • Active, unrestricted state license and DEA license.
  • Board certification in a relevant specialty.
  • Current state driver's license and proof of auto insurance.
  • Strong technical skills and experience with electronic medical records (EMRs).
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Preferred Qualifications
  • Experience working in home-based primary care or a related setting.
  • Knowledge of gerontology and the unique needs of older adults.
  • Experience with population health management and care coordination.
