Key Responsibilities- Provide comprehensive primary care services to patients in their homes, including wellness visits, physical exams, chronic condition management, and minor injury treatment.
- Conduct thorough assessments, diagnose illnesses, and develop treatment plans to improve patient outcomes.
- Prescribe medications, order lab tests, and coordinate care with other healthcare professionals as needed.
- Build strong relationships with patients and their families, providing education and support to promote healthy lifestyles and disease management.
- Collaborate with our interdisciplinary care teams to ensure seamless care coordination and communication.
- Bachelor's degree in Nursing or related field.
- Minimum 2 years of clinical experience, preferably in primary care or home health care.
- Active, unrestricted state license and DEA license.
- Board certification in a relevant specialty.
- Current state driver's license and proof of auto insurance.
- Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Master's degree in Nursing or related field.
- Experience in urgent care, primary care, or internal medicine.
- House call experience or familiarity with home health care settings.
