**Job Summary:**
We are seeking a highly skilled Clinical Social Worker Health Care Facility Surveyor to join our team at Greenlife Healthcare Staffing. As a Clinical Social Worker Health Care Facility Surveyor, you will be responsible for conducting surveys of health care facilities to ensure compliance with regulatory standards.
**Responsibilities:**
* Conduct surveys of health care facilities to assess compliance with regulatory standards
* Evaluate the quality of care provided to residents of long-term care facilities
* Identify areas for improvement and provide recommendations for corrective action
* Collaborate with facility staff to implement changes and improve quality of care
* Maintain accurate records of survey findings and recommendations
**Requirements:**
* Master's degree in Social Work
* Successful completion of CMS's Long-Term Care Basic Training and SMQT certification
* At least two years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities
* Current licensure to practice as an LCSW
* Demonstrated history of independent decision-making skills to direct and effectively manage the survey process
* Ability to set priorities independently and collectively in performing survey tasks
* Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts
