Job Summary

We are seeking a skilled Community Health Worker I to join our team at UTHealth Houston. As a Community Health Worker, you will play a vital role in supporting patients in finding community resources and referrals as needed. Your previous experience in multispecialty settings will be highly valued.

As a member of our team, you will work closely with physicians, nurses, and clinical care team members to provide patients with internal and external resources, and deliver patient education to address and improve preventative and clinical quality care outcomes.

Key Responsibilities:

  • Establish and maintain rapport with physicians, clinical and business staff, and patients.
  • Conduct patient pre-visit planning procedures to identify gaps in care and report collected data and findings to the clinical care team.
  • Gather and document demographic, insurance, health, and social needs of patients to address barriers to care.
  • Help patients utilize resources in their area and assist with applications for eligible programs.
  • Educate patients and their families on clinical care team roles, expectations of patient-centered medical home activities, and the importance of preventative healthcare.
  • Motivate patients to be active, engaged participants in their healthcare.

Requirements:

  • Texas Department of State Health Services (DSHS) – Certified CHW
  • Bilingual (English/Spanish) preferred
  • Ability to perform complex and diverse administrative duties within the CHW scope of work.
