Job Title: Home Health Speech Language Pathologist

We are seeking a highly skilled and compassionate Home Health Speech Language Pathologist to join our team at FC Compassus LLC. As a key member of our interdisciplinary team, you will be responsible for providing high-quality speech therapy services to patients in their homes, promoting independence, and improving communication skills.

Responsibilities:
  • Develop and implement individualized speech therapy plans of care for patients with speech and language disorders.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Provide education and training to patients, families, and caregivers on communication strategies and techniques.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements:
  • Master's degree in Speech Language Pathology or related field.
  • Active and unencumbered Speech Language Pathologist license in state(s) of employment.
  • Minimum of one (1) year of experience in a home health setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
