We are seeking a skilled Admissions Assessment Nurse to join our team. As a key member of our nursing staff, you will be responsible for providing high-quality resident care services and increasing their satisfaction by facilitating and managing all aspects of the nursing admission process.

Responsibilities:
  • Assess patients/residents upon admission for appropriateness of admission, coverage, and level of care determination
  • Communicate with internal staff regarding coverage and contract details
  • Proactively work with a multidisciplinary team to manage care, costs, and discharge planning
Qualifications:
  • Registered Nurse with a valid current licensure with the State Board of Nursing in the state of Minnesota
  • Bachelor's degree with previous admissions/or intake experience preferred
  • Long-term care, acute care, or subacute experience, preferred
  • Knowledge of MDS, payer systems, and contracts (Medicare, HMOs, Managed Care, Third Party Payer Systems)
