We are seeking a highly motivated and experienced Social Worker to join our team at ChristianaCare. As a Social Worker in our Acute Care department, you will play a critical role in providing compassionate and high-quality care to our patients and their families.
Key Responsibilities- Collaborate with the Care Management Triad Team to develop and implement patient care plans that address psycho-social barriers and support discharge needs.
- Work closely with RN Case Managers and Utilization Management to ensure patient progression and appropriateness of care.
- Provide education and support to patients and families to promote self-care and independence.
- Participate in interdisciplinary teams to develop and implement care plans that address the physical, emotional, and social needs of patients.
- Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school.
- Licensed CSW or MSW as required by the state of Delaware.
- At least 2 years of experience in acute care social work, preferably in a maternal-child setting.
- CCM certification preferred within 18 months of eligibility.
