Job Summary

We are seeking a skilled Clinical Social Worker to join our team as a Case Manager. In this role, you will work with members of a defined population to facilitate the coordination of appropriate, cost-effective services that are consistent with their plan of care. Your goal will be to help them achieve their optimal level of independence and enhance their quality of life.

Key Responsibilities
  • Identify and assess the needs of our members, providing psychosocial support and connecting them with community resources.
  • Develop and implement individualized plans of care, working closely with the healthcare team to ensure seamless coordination of services.
  • Manage and coordinate assigned caseloads, ensuring timely and effective delivery of care.
  • Analyze patient and program outcomes to identify areas for improvement and optimize case management activities.
  • Provide education and support to members, families, and caregivers, as well as healthcare professionals and community agencies.
Requirements
  • Master's degree in social work (MSW) required.
  • Minimum three years of clinical experience and two years in case management.
  • Licensed Clinical Social Worker (LCSW) credentials required within six months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment.
  • Knowledge of community systems and resources in the defined service area.
  • Knowledge of regulatory issues for the Mid-Atlantic area.
