About the Role

We are seeking a skilled Social Worker to join our team at Texas Health Arlington Memorial Hospital. As a Social Worker, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay and beyond.

Key Responsibilities

As a Social Worker, you will be responsible for:

  • Conducting thorough assessments of patients' social and emotional needs
  • Developing and implementing individualized care plans to address patients' needs
  • Collaborating with the interdisciplinary team to ensure seamless transitions of care
  • Providing education and support to patients and their families
  • Identifying and addressing potential barriers to care


Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Work (MSW) or a related field
  • At least 3 years of experience in acute care hospital or medical social work
  • Experience with discharge planning and care management is highly preferred
  • Licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) is required
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification is preferred
