- Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination.
- Observe and report any changes in resident health, escalating concerns to supervisors as needed.
- Collaborate with the care team to develop and implement individualized care plans.
- Provide emotional support and comfort to residents and their loved ones.
- High School Diploma or equivalent.
- Minimum 2 years' experience in a hospital or clinical setting.
- Associates degree or higher in nursing or a related field (preferred).
- CPR/First Aid Certification (desired).
- Valid, unrestricted LPN certification or State/Local equivalent.
