Job Title: Medication Aide - Harmar Place

At United Church Homes, Inc., we are seeking a highly skilled and compassionate Medication Aide to join our team at Harmar Place. As a Medication Aide, you will play a vital role in providing exceptional care to our residents, ensuring their physical and emotional well-being.

Job Summary:

The Medication Aide will work under the supervision of the Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide resident care in accordance with the Ohio Nurse Practice Act. This includes administering medications, performing ADL and rehabilitation activities, and documenting all entries on flow sheets and other documents.

Key Responsibilities:
  • Participate in and receive nursing report upon reporting for duty
  • Document, at time of service, all medications and treatments administered
  • Document in EMR accurately and timely
  • Perform activities related to medications and treatments in accordance with community policies and procedures
  • Maintain control of drugs in medicine cart (excluding locked scheduled drugs); monitor for discrepancies and report promptly to nurse
  • Assist residents at mealtimes
  • Assist with resident care, including bathing, grooming, hygiene, and placement of adaptive equipment
  • Explain procedures to residents before administering, drape residents for examination and treatment
  • Keep residents dry (change clothing, linen, etc., when wet or soiled)
  • Assist residents with bowel and bladder functions
  • Empty urinary drainage bags and record amount in record; report any irregularities to nurse
  • Assist with lifting, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., keeping with specific resident safety needs; assist residents in ambulation, with or without self-help devices, as instructed
  • Perform restorative and rehabilitative procedures, as instructed
  • Ensure call lights are within reach of residents and answer call lights promptly
  • Ensure residents' personal care needs are being met in accordance with residents' wishes
  • Report all changes in residents' condition to supervisor as soon as practical
  • Change bed linens and keep linens tight to avoid wrinkles from forming under residents; handle linens according to infection control guidelines
  • Maintain cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, linen closets
  • Assist residents in preparation for and transporting to appointments, activities, and social programs
  • Use creative problem-solving to meet the needs of the resident, within scope of responsibility
  • Provide post-mortem care, as instructed
  • Perform required special nursing care duties, including identifying and reporting abnormal skin conditions
  • Ensure residents are positioned and prepared appropriately for meals; serve food and assist with feeding as necessary; ensure each resident is properly hydrated; assist in the delivery and documentation of all snacks and supplements
  • Accurately record all entries on flow sheets, I and O's, and other documents at point of care as required
  • Participate in care conference, as indicated, and follow individual resident care plans
  • Perform all admission, discharge, and transfer duties, as required
  • Maintain confidentiality of necessary information
  • Utilize supplies and equipment properly and without waste
  • Report observations concerning structural, equipment, and furniture defects and malfunctioning to appropriate personnel
  • Handle unusual occurrences calmly and logically to maintain continuity of duties
  • Maintain appropriate, effective communication with residents, and with residents' family or other significant relationships, and ensure departmental staff does the same
  • Follow all appropriate safety and security guidelines, care procedures, and protocol for residents in the Dementia Care Unit
  • Participate in relevant educational and training activities, as appropriate
  • Participate in special activities, as necessary (e.g., safety committee; mentoring/orientation/new hires; etc.)
  • Perform all other duties, as assigned or directed
Requirements:
  • Successful completion of a state-approved Nursing Assistant program and Ohio Board of Nursing approved MA-C training
  • Must obtain and maintain mandatory, state, and federal requirements and certifications for practice or occupation
  • Ability to assist patients during therapy
  • Special care/dementia care employees will have additional training in caring for and treating dementia residents
  • Ability to communicate effectively with residents and nursing staff and have strong interpersonal skills
  • Ability to read, write, understand, and speak the English language
  • Ability to use computer for charting purposes
  • Must be able to operate the following equipment: Sit to Stand, Hoyer Lift, Gait belts, tub chairs
  • Must be dependable
  • Must be compassionate to residents and have a true desire and commitment to assist them
  • Must be able to work with elderly residents, families, and other healthcare staff members
  • Must participate in Continuing Education
  • Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook
