Responsibilities:

About the Position:


During a visit that can last up to one hour, Matrix providers assess and observe a member's current health status, medical history, medication adherence, social environment, and other potential risks.

This approach provides unparalleled insight into a member's overall health and well-being, which can be challenging to capture during standard office visits.

The Matrix Comprehensive Health Assessment enhances the quality of care and allows us to address multiple care gaps in a single visit.

Type:
Travel Nurse Practitioner - PRN

Location:
Short-Term PRN Travel assignments within the State of California

Hours:
Days. Flexible schedule. No on-call or evening shifts.

Salary:
$125 per completed assessment - Daily Per Diem and Travel Reimbursement

Benefits:
PRN providers are non-benefited except for limited Voluntary Benefits.

Matrix Provider – What to Expect:
Make a difference in people's lives by conducting Adult/Geriatric Assessments, gathering medical history, providing diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation according to Matrix guidelines and protocols.

Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and any relevant ancillary medical personnel as appropriate.

Engage with Primary Care Physicians (PCP) on patient education and provide follow-up care.

Be part of a nationwide network of advanced practice providers to build and maintain professional relationships.

Services may be provided in various settings, including Home Visits, Skilled Facility Visits, and Telehealth Visits.

THIS IS NOT A FULL-TIME POSITION – It is PRN/COVERAGE with no guaranteed hours.

Qualifications:

Must Haves:
Master's Degree required OR equivalent experience and successful completion of NP licensure.

Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.

Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty.

Current BLS, ACLS, or CPR certification.

A minimum of 1 year of experience as a Nurse Practitioner.

Strong computer skills and familiarity with employee health/medical record software.

Excellent verbal and written communication skills with patients, clients, and colleagues.

Ability to adapt and remain flexible in a changing environment.

Travel Requirements:
Ability to travel, possess a valid state driver’s license, and have adequate automobile insurance coverage for the state of residence.

Required 20-mile driving radius.
