We are seeking a highly motivated and compassionate Care Coordinator to join our team at Interim HealthCare of Fenton, MO. As a Care Coordinator, you will play a vital role in providing exceptional care and support to our clients in the comfort of their own homes.
Key Responsibilities:
- Assist in providing client care coordination and serve as a single point of contact for care professionals, clients, families, and providers.
- Receive referral calls, document job orders, and assist in staffing orders with qualified personnel.
- Conduct home evaluations and visits to assess client needs.
- Promote agency services through education to prospective and existing patients/clients.
- Assist with supervision of all care professionals and complete on-call rotation/responsibilities with office staff.
- Create and maintain staff schedules, and provide staffing for sick leave, vacation, and long-term leave.
- Schedule client appointments/visits according to care plans and staff availability.
- Enter scheduling data, create schedules, and contact care providers and clients regarding day-to-day changes.
- Discern client services required as outlined in agreements, urgent requests, and care plans.
- Maintain staff and client information in the database and software system.
- Identify and resolve situations and problems, taking action within authority limits to prevent reoccurrence of any negative issues.
- Provide input to management on improving processes and procedures, developing recognition and reward programs, and becoming more cost and time efficient.
Requirements:
- High school diploma or equivalent required.
- Previous experience in care coordination or a related field preferred.
- Excellent communication and problem-solving skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Knowledge of home care services and regulations.
