Nursing Case Manager

The Clinical Care Coordinator will work closely with our clinical team to coordinate client care, manage medical records, and ensure compliance with regulatory requirements. This is an excellent opportunity for a detail-oriented individual who is passionate about delivering exceptional patient care.
Key Responsibilities:
- Coordinate client care by assisting with case orientation, care insight review, and care coordination efforts.
- Manage medical records, including tracking order completion and ensuring signatures are captured.
- Coordinate scheduling for post-fall and resumption of care visits.
Required Skills and Qualifications:
- Graduate of an accredited school of professional nursing.
- Homecare experience preferred.
- Current and valid DC LPN license.
Work Environment and Physical Demands:
- Able to perform tasks involving physical activity, which may include heavy moving, bending, lifting, and extensive self-positioning.
- Continual or almost continual standing may be required.
