- Collaborate with community nurses, caregivers, and medication personnel to ensure seamless care delivery.
- Document and report care provided and resident observations, including any unusual or significant changes.
- Maintain a working knowledge of state regulations, policies, and procedures to ensure compliance.
- Strong clinical skills and knowledge of healthcare regulations.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a team and adapt to changing priorities.
- Flexibility to work varied shifts, including overnights.
