Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team at The City of Providence. As a Community Health Worker, you will play a vital role in promoting health equity and improving the well-being of our community members.

Key Responsibilities
  • Engage with community members to identify areas of need and provide referrals to appropriate services.
  • Implement Social Determinants of Health and other screening tools to assess community needs.
  • Provide individuals with Personal Protective Equipment (PPE) and COVID-19 prevention materials upon request.
  • Connect individuals to community-based vaccination events and provide support as needed.
  • Complete home/community visits to support individuals and lead community health worker networking meetings.
Requirements
  • Associates or Bachelor's degree in a related field preferred.
  • Certified Community Health Worker or ability to be certified within first 12 months of employment.
  • Bilingual in Spanish required.
  • Valid driver's license and reliable transportation.
  • Ability to document in and navigate an Electronic Medical Record (EMR).
  • Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.
  • Ability to provide services virtually, in resident's homes, and community locations.
  • Ability to have a flexible schedule to include evening shifts and occasional weekends.
