HouseCalls Clinician Opportunity

Optum is seeking a skilled NP or PA to join our HouseCalls team in Lane County, OR. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date, with licensure unencumbered and free of any open/unresolved disciplinary actions.
  • Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
