Essential Functions:
- Prepare and administer medications to residents following established guidelines.
- Conduct narcotic counts at the start and end of each shift.
- Order medications from the pharmacy in compliance with community policies.
- Review Medication Administration Records (MAR) for discrepancies as per community standards.
- Communicate with residents' families regarding medication needs when necessary.
- Remove expired or discontinued medications and prepare them for safe disposal.
- Administer treatments, monitor vital signs, and assist with resident weight checks.
- Document changes in residents' conditions and unusual occurrences accurately.
- Ensure completion and filing of lab work, physician orders, and medication orders according to community protocols.
- Assist in scheduling medical appointments for residents as required.
- Support new resident admissions in accordance with community policies.
- Assist residents with personal care tasks such as bathing, grooming, dressing, and toileting.
- Help residents with daily living activities as outlined in their individual care plans.
- Participate in basic cleaning and maintenance tasks as needed.
- Respond promptly to incidents while maintaining the dignity of residents.
- Provide assistance to residents who have fallen, ensuring their safety and comfort.
- Accompany residents to and from dining areas as necessary.
- Collect laundry from residents' apartments and transport it to the laundry facilities.
- Assist in serving meals and beverages, and taking meal orders as needed.
- Respond to call lights with patience and understanding, adhering to company guidelines.
- Engage with residents empathetically, addressing their concerns and following up appropriately.
- Conduct regular rounds to ensure resident safety and provide assistance as required.
- Notify Health Services Director, physicians, and families of any changes in residents' conditions or medication needs.
- Participate in meetings, training sessions, and in-services as required by management.
- Maintain confidentiality of residents' personal information and uphold their rights.
- Communicate positively with residents, families, visitors, and team members using excellent interpersonal skills.
- Provide tours to prospective residents and families, showcasing how their individual needs can be met.
- Exhibit a professional appearance and demeanor that fosters a nurturing environment for residents, families, and guests.
- Ensure outstanding customer service by maintaining open communication and addressing the needs of residents, families, and staff.
- Coordinate with the Culinary Services team to ensure adequate coverage for meal service, including taking meal orders and assisting with dining room preparations.
- Demonstrate a commitment to service by embodying the mission of Lifestream.
Qualifications:
- Strong interpersonal skills for effective communication with residents, staff, and families.
- Genuine compassion and care for the elderly and disabled individuals.
- Knowledge of hazardous materials and safety protocols.
- Ability to work independently and prioritize tasks in emergency situations.
- Capability to manage stressful interactions with residents, families, and staff.
- Proficient in safe medication administration practices.
Education and Experience:
A high school diploma or equivalent is preferred. Certification in medication administration and CPR/First Aid is required per state regulations.
Working Environment:
This position requires moderate physical activity, including handling objects up to 50 pounds occasionally and up to 20 pounds frequently. The ability to stoop, bend, and reach is necessary, as well as the capacity to lift residents exceeding 100 pounds with assistance.
