Job Summary

Pathwell Health is seeking a highly skilled and compassionate Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in assessing and evaluating patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments.

Key Responsibilities
  • Assessment and Evaluation: Conduct comprehensive assessments and evaluations of patient care needs, identifying areas of strength and weakness in communication, swallowing, and cognitive function.
  • Treatment Planning: Develop and implement individualized treatment plans to address patient needs, working collaboratively with the healthcare team to ensure comprehensive care.
  • Interventions and Therapy: Provide evidence-based interventions and therapy to patients, focusing on improving communication, swallowing, and cognitive function.
  • Progress Monitoring: Regularly monitor patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
  • Communication and Collaboration: Maintain open and effective communication with patients, families, and healthcare team members, ensuring seamless care coordination.
Requirements
  • Master's Degree: Possess a Master's degree in Speech Therapy or Speech-language Pathology from an accredited institution.
  • Licensure: Be currently licensed in Speech Therapy in Virginia, with a valid driver's license and car insurance.
  • Certifications: Hold current CPR certification and be proficient in utilizing electronic devices for tasks such as timekeeping, in-servicing, and documentation.
  • Language Skills: Possess strong leadership skills, with the ability to work collaboratively with a large team, and be bilingual (Spanish) preferred.
Work Environment

The Speech Language Pathologist will work in a home health setting, visiting patients in their homes to provide therapy services. The ideal candidate will possess strong clinical assessment, organizational, and communication skills, with the ability to handle multiple assignments simultaneously.
