Home Health Physical Therapist Assistant

CenterWell Home Health specializes in delivering personalized, comprehensive home care to patients managing chronic conditions or recovering from injuries, illnesses, or hospitalizations. Our dedicated care teams, comprising nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, collaborate to help patients rehabilitate, recover, and regain their independence.
Schedule and CompensationWe offer a competitive salary with the estimated annual pay range of $57,700 - $79,500. The compensation is based on full-time employment (40 hours per week) and may vary depending on individual skills, knowledge, experience, education, certifications, and geographic location.
Job DescriptionAs a Home Health Physical Therapist Assistant, you will be responsible for administering the physical therapy treatment plan developed by the Physical Therapist. Your duties will include:
- Administering non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients;
- Accurately documenting daily, weekly, or monthly records of clients' progress and submitting clinical paperwork to the office on time each week;
- Attending educational meetings and contributing to department in-services;
- Notifying physical therapists or Executive Directors of equipment failures, lack of supplies, staff or client changes, and any other departmental matters;
- Scheduling clients and performing various office tasks;
- Applying concepts of infection control and universal precautions in performing patient care activities;
- Accepting clinical assignments consistent with your education and competence to care for patients;
- Promoting compliance with all fiscal intermediaries and/or other third-party payors through education, coaching, and assistance as necessary.
To excel in this role, you should possess the following qualifications:
- An Associate degree in Physical Therapy from an American Physical Therapy Association;
- A current state license to practice Physical Therapy;
- A valid driver's license, auto insurance, and reliable transportation;
- Proof of current CPR certification;
- Minimum two years of experience as a Physical Therapy Assistant;
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population;
- Self-directed, enthusiastic, and open to constructive feedback;
- Good verbal and written communication and organization skills;
- A professional image/attitude for patients, families, clients, coworkers, and others;
- The ability to work a flexible schedule and travel locally;
- Maintaining confidentiality;
- Meets mandatory continuing education requirements of CenterWell Home Health and licensing board;
- Ability to read, write, and speak fluent English;
- Good and regular attendance.
