LPN - Family Practice Nurse

Job Summary:
Excela Health is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Family Practice team. As an LPN, you will play a vital role in providing high-quality patient care, working closely with our healthcare team to deliver exceptional outcomes.
Responsibilities:
- Deliver direct and indirect patient care in accordance with the LPN Practice Act, hospital and/or physician practice policies, procedures, protocols, and other professional standards of care.
- Collect patient health care data, including vital signs, weight, chief complaint, and pain assessment, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient.
- Administer medications/immunizations using appropriate routes and techniques specific to the patient's age.
- Comply with completeness, accuracy, and timeliness of documentation.
- Provide education and instruction to patients and families within your scope of practice.
- Assist in assessing the patient's educational needs and identifying educational barriers.
- Maintain a clean and safe environment, ensuring that supplies in patient care areas are replenished daily.
- Perform quality control testing for all point-of-care testing per policy.
- Participate in non-punitive medication reporting and adhere to infection control processes.
- Act as a patient advocate and perform effectively in the role of preceptor/mentor to new hires and/or students.
Requirements:
- Graduate of an accredited School of Practical Nursing.
- Current licensure to practice as a Licensed Practical Nurse in the State of Pennsylvania.
- Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
- Act 33 with renewal, Act 34 with renewal, and Act 73 FBI Clearance.
Working Conditions:
This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The incumbent will be scheduled based on operational need, including rotating shifts, standby, on-call, and travel between Excela Health locations.
