We are seeking a highly skilled and compassionate Primary Care Physician to join our team at WelbeHealth. As a Geriatric Medicine Specialist, you will be responsible for providing comprehensive medical care to our elderly patients, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Conduct in-person comprehensive assessments of patients and develop care plans in collaboration with our interdisciplinary team.
- Oversee the care of patients in nursing homes and hospitals, including admits and discharges, and coordinate with medical specialists as needed.
- Provide direction and support to Nurse Practitioners and other members of the interdisciplinary team.
- Collaborate with community-based physicians and our interdisciplinary team to ensure seamless care transitions and effective discharge planning.
- Participate in rotating night and weekend call to ensure 24-hour coverage.
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate Degree in Medicine.
- Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics (preferred).
- Active California Physician License without Restrictions.
- Active Drug Enforcement Administration (DEA) registration.
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants.
- Reliable means of transportation.
- State CDS Certification and DEA Registration.
- Competitive salary range: $250,000 - $280,000 per year, plus bonus and equity.
