Job Summary

We are seeking a Full-Time Registered Nurse (RN) to join our team in Little Compton, Rhode Island. As a Home Health RN, you will be responsible for delivering high-quality care to our patients, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including administration of medication, wound care, and treatments.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient care and progress accurately and thoroughly, using electronic medical records.
Requirements
  • Graduation from an accredited and approved nursing program.
  • Current licensure as a Registered Nurse (RN) in the state of Rhode Island.
  • Experience in home health care or a related field preferred.
