Job Summary:We are looking for a compassionate and experienced Registered Nurse to provide comprehensive nursing care to patients in their homes. The ideal candidate will have a strong background in community health nursing and be able to work independently with minimal supervision.
Responsibilities:- Conduct comprehensive health and psychosocial evaluations of patients to identify their needs and develop individualized care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Coordinate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

- Current licensure as a Registered Nurse in the state of Florida.
