We are currently seeking a Registered Nurse or Licensed Practical Nurse to join our Hospice division. This RN or LPN role offers flexibility and is essential to our mission.

RN/LPN Responsibilities

  • Conduct ongoing assessments of the patient's physical and psycho-social status
  • Develop and initiate a care plan in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician
  • Provide education and interventions to effectively manage the patient's pain and other physical symptoms, while assessing the patient/family's response to treatment
  • Ensure the availability of necessary equipment, supplies, medications, and services
  • Adjust the IDG care plan as the patient's/family's needs evolve
  • Document patient/family services accurately and in a timely manner
  • Regularly participate in IDG meetings

RN/LPN Requirements

  • Active, unrestricted RN/LPN license in the State of Michigan
  • Minimum of one (1) year of relevant experience as an RN or LPN (required)
  • Preferred: One (1) year of management experience
  • Preferred: One (1) year of home health experience
  • Current CPR Certification
  • Preferred: Community health and a broad range of Medical-Surgical experience
  • Reliable transportation with a valid state driver's license and current car insurance
