Job Description

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Monogram Health.

As a key member of our integrated Care Team, you will work closely with a Registered Nurse and a Social Worker to deliver personalized, compassionate medical care to patients with chronic kidney and end-stage renal disease.

Your primary responsibilities will include conducting comprehensive annual wellness exams, counseling and educating patients and families about benefits and programs available to help them live healthier lives, and documenting patient information in a timely manner.

You will also be responsible for coordinating care with primary care providers, specialists, and ancillary services, and maintaining quality of care standards as defined by the practice.

Key Responsibilities:

  • Conduct comprehensive annual wellness exams and document patient information
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Maintain quality of care standards as defined by the practice
  • Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life

Requirements:

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work without direct supervision and practice autonomously
  • Access to transportation, a valid driver's license, and car insurance
