At Monogram Health, we are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our integrated Care Team, you will play a vital role in delivering personalized medical care to patients with chronic kidney disease and end-stage renal disease.
Key Responsibilities- Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to help them live healthier lives.
- Document patient information, including chief complaint, diagnosis, past medical history, and plan of care.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Prescribe medications, order tests, and collaborate with patients' Monogram physicians.
- Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work without direct supervision and practice autonomously.
- Access to transportation, a valid driver's license, and car insurance.
