Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a Nurse Practitioner, you will provide primary and consultative professional care to patients in a variety of care settings, focusing on geriatric and seriously ill populations. You will work collaboratively with physicians and other healthcare professionals to develop and implement individualized care plans that promote comfort, quality of life, and optimal health outcomes.

Key Responsibilities
  • Conduct thorough medical evaluations of patients in their home environment, including geriatric and seriously ill populations.
  • Determine follow-up assessments based on patient need and best practice guidelines.
  • Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, physician, family, facility, and other personnel involved in patient care.
  • Assess health status by obtaining relevant health and medical history.
  • Perform physical exams based on patient age and history, in collaboration with a collaborating physician.
  • Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient clinical problems and healthcare needs.
  • Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges.
  • Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
  • Develop treatment plans in collaboration with patients and caregivers.
  • Refer patients to physicians as appropriate for consultation or specialized health resources and treatment.
  • Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.
  • Provide patients and families with an understanding of diagnosis and disease progression, while decreasing unnecessary emergency room and hospital visits.
  • Hold case conferences with families when appropriate to advance the care plan.
  • Meet minimum productivity requirements based on practice standards.
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
  • Master's or doctoral degree as a Board-Certified Nurse Practitioner.
  • Current State License as a Registered Nurse and Nurse Practitioner.
  • Nursing skills as defined by accepted nursing standards.
  • Sit, stand, bend, lift, and move intermittently.
  • Must be able to lift at least 35lbs and bear the weight of an average adult effectively.
  • Current and valid driver's license.
  • Reliable transportation and valid automobile insurance coverage.
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled-nursing facility settings.
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
  • Ability to handle multiple priorities, documentation requirements, and deadlines.
Preferred Qualifications
  • 2 years clinical experience in a post-acute or as a Nurse Practitioner.
  • Experience with geriatric patients.
  • Familiarity with Aprima EMR.

Office hours: 8:00 AM - 5:00 PM, Monday through Friday
