Key Responsibilities- Conduct comprehensive physical examinations, compile patient medical data, and order diagnostic tests as needed.
- Develop and implement patient management plans, record progress notes, and ensure continuity of care.
- Instruct and counsel patients on health maintenance, addiction treatment, and prescribed therapeutic regimens.
- Prepare patients for discharge, including medications and instructions, and ensure a smooth transition to post-care.
- Participate in Performance Improvement Initiatives and contribute to a culture of excellence.
- RN with MSN and Current NY State Nurse Practitioner license.
- Experience in detoxification, addiction medicine, or primary care preferred.
- DEA License and Professional Liability Insurance required.
- CPR certification and ACLS certification preferred.
