Job Summary

The Registered Nurse Case Manager at PACE Southeast Michigan plays a vital role in delivering high-quality care to our participants. As a skilled nurse, you will assess the needs of our seniors with complex health issues and develop personalized plans to enhance their functional capacity and independence.

Duties and Responsibilities:

  • You will conduct thorough assessments of our participants' needs and create tailored care plans to address their physical and psychosocial requirements.
  • Collaborate with our multidisciplinary team to ensure seamless coordination of care and optimal outcomes for our participants.
  • Provide education and training to our participants and their caregivers on disease prevention and management.
  • Administer medications and treatments as ordered by our physicians, while monitoring participants' responses and reporting any changes in their status.
  • Maintain accurate and timely records of participants' progress, including charting and documentation of data and outcomes.
  • Advocate on behalf of our participants to ensure their rights are respected and their concerns are addressed.

Requirements:

  • A valid RN license in Michigan and a minimum of one year of experience working with frail or elderly populations.
  • Ability to lead and direct other nursing staff, including licensed and non-licensed personnel.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Willingness to participate in ongoing professional development and continuing education to stay current with best practices in gerontological nursing.

Estimated Salary Range: $65,000 - $85,000 per year, depending on experience and qualifications.
