About the Role

PACE Southeast Michigan is seeking a highly skilled Registered Nurse to serve as a Case Manager. This role is an excellent opportunity for a seasoned RN to utilize their expertise in nursing practice, assessment, planning, implementation, and evaluation of care for frail elders with complex needs.

The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to lead or direct licensed and non-professional nursing staff in the delivery of care. The Registered Nurse Case Manager will be responsible for assessing participants' needs, developing care plans, implementing interventions, and evaluating outcomes.

Responsibilities:

  • Provide comprehensive nursing care to participants, including assessments, care planning, and implementation of interventions.
  • Lead or direct licensed and non-professional nursing staff in the delivery of care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Evaluate participants' progress toward achieving care plan objectives.
  • Communicate effectively with participants, families, and healthcare team members.

Requirements:

  • Bachelor's degree in Nursing (BSN) preferred.
  • Current Michigan licensure as a Registered Nurse.
  • At least one year of experience working with frail or elderly populations.
  • Strong communication and leadership skills.
  • Able to establish and maintain effective relationships with participants, families, and healthcare team members.

Estimated Salary Range: $62,000 - $72,000 per year. Benefits include health insurance, paid time off, and opportunities for professional growth and development.
