Role Description

Under the guidance of the Director of Nursing Services, the MDS Coordinator will oversee and coordinate the Interdisciplinary Team (IDT) to ensure timely completion of MDS assessments in compliance with State and Federal regulations.

Qualifications
  • Active State Nurse License
  • Strong written and verbal communication skills
  • Excellent organizational and administrative abilities
  • Current CPR certification preferred
  • Minimum of two years of RN experience in long-term care
  • Previous supervisory experience
Key ResponsibilitiesClinical Duties
  • Manage the Medicare/MDS resident assessment process.
  • Ensure timely completion of MDS assessments by the Interdisciplinary Team.
  • Coordinate the development, implementation, and evaluation of care plans.
  • Administer treatments, medications, and nursing interventions as outlined in the care plan or physician orders.
  • Provide nursing care in adherence to infection control standards.
  • Follow safety protocols during nursing care delivery.
  • Initiate emergency measures as necessary, in accordance with policy and nursing standards.
Administrative Duties
  • Facilitate the exchange of essential information for quality resident care.
  • Maintain documentation as required by Federal and State regulations and company policies.
  • Participate in all assigned meetings and training sessions.
