We are seeking a compassionate and skilled Case Manager to join our Recovery Starship team. As a primary support and case manager, you will work directly with individuals who have been faced with multiple acute psychiatric hospital admissions and have difficulty maintaining a residence and their independence in the community.



About the Role

As a Case Manager, you will provide personalized support and guidance to help individuals achieve their goals and live independently in their chosen community. You will work closely with a multidisciplinary team to develop and implement effective treatment plans that address the unique needs of each individual.



Key Responsibilities

The successful candidate will have the following key responsibilities:





  • Provide direct support and case management services to individuals with mental illness and other co-occurring disorders.

  • Assess and identify the needs of each individual and develop personalized treatment plans.

  • Work collaboratively with the multidisciplinary team to implement treatment plans and achieve individual goals.

  • Provide transportation to appointments and groups, as needed.

  • Maintain accurate and up-to-date records of individual progress and treatment plans.

Requirements

To be successful in this role, you will need to have:





  • A high school graduation or G.E.D. equivalent and two years of direct services in mental health or community services.

  • A valid and current driver's license, as well as personal vehicle insurance with your name listed as a driver.

  • The ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.

  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and multidisciplinary teams.
