Overview:
As a Clinical Pharmacist, you will be responsible for overseeing and managing comprehensive medication therapy to enhance the effectiveness of pharmaceutical care.
Key Responsibilities:
- Assess, interpret, and analyze medication regimens to identify, prevent, and resolve potential or existing medication-related issues.
- Deliver patient-centered care by initiating, adjusting, and monitoring medication therapies through approved methods, including therapeutic interchange and collaborative practice agreements.
- Provide drug information and educational resources to healthcare professionals, students, and patients.
- Collaborate with the healthcare team to design, implement, and assess quality improvement initiatives related to medication management.
- Demonstrate critical thinking, sound judgment, and the ability to operate with minimal supervision in a fast-paced environment.
- Licensed Pharmacist
- Education: Bachelor's Degree in Pharmacy from an accredited institution.
- Licensure: Valid state licensure as a Pharmacist.
- Experience: No prior experience required.
- Certification: BLS certification through the American Heart Association or the American Red Cross CPR for the Professional Rescuer, valid upon start date.
