InGenesis is in search of a dedicated Licensed Vocational Nurse (LVN) to join our team as a Wellness Case Manager. In this position, you will assess, strategize, and deliver case management services, health education, and counseling for patients dealing with intricate chronic conditions such as Diabetes, Hypertension, Congestive Heart Failure, and Asthma, adhering to BACH's Chronic Care model.

Key Responsibilities:- Deliver a range of professional case management services.
- Manage the Chronic disease registry and keep patient information current.
- Schedule appointments and send reminders for consultations and preventive care.
- Monitor the registry for overdue laboratory tests.
- Create and execute a tracking system for referrals.
- Assist in determining eligibility.
- Maintain ongoing communication with clinical personnel.
- Perform additional duties as assigned.
- High School Diploma.
- Completion of an LVN program.
- Current LVN license in California.
- 1–2 years of experience in a medical setting.
- Active phlebotomy certification.
- Access to reliable transportation.
- Proficiency in Microsoft Office and internet usage.
- Associate or Bachelor's degree in a related field.
- Experience in a community health center or public health environment.
- Experience working with patients suffering from Chronic Diseases.
- Experience in outreach and presentations.
Physical Requirements:
- Ability to sit, walk, and stand in a standard office environment.
- Manual dexterity for operating office equipment.
- Occasional exposure to outdoor elements.
- Capability to present information to groups.
- Reading and writing skills for record-keeping.
- Potential exposure to blood, requiring OSHA training.
