**Job Summary:**

We are seeking a highly skilled Nurse Practitioner to join our team at Oak Street Health. As a Nurse Practitioner, you will provide exceptional primary care to our patients, working independently and collaboratively with physicians and other healthcare professionals.

**Responsibilities:**

* Provide longitudinal care and build meaningful patient relationships
* Conduct office visits for routine and acute issues
* Administer Annual wellness visits and health risk assessments
* Lead population health initiatives and coordinate with the Care Team
* Assist with phone triage and outreach
* Educate patients on their health conditions and care plans
* Conduct home visits as needed

**Requirements:**

* Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
* National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
* Active, non-probationary state Nurse Practitioner license
* Active DEA license
* US Work Authorization
