  • Monitor clients' progress and ensure compliance with treatment plans.
  • Provide education and support to clients to promote self-management and independence.
  • Make referrals to outside sources as needed to ensure comprehensive care.
  • Review data for trends and provide ongoing monitoring to ensure quality care.

Requirements:

  • Bachelor's degree in a Human Services field.
  • 2+ years of community-based mental health case management experience.
  • Experience working with minors and vulnerable populations.
  • Proficient in Microsoft products and electronic medical records.
  • Access to secure internet for remote work.
  • Licensure or eligibility to sit for licensure within 24 months.

Preferred Qualifications:

  • Limited Licensed Social Worker or equivalent.
  • Certified Case Manager or ability to obtain certification.
  • Background in community-based care or managed care.
  • Experience in discharge planning and healthcare disparities.
  • Knowledge of utilization review and healthcare disparities.
