Advanced Heart Failure Care Specialist

Overview
The Dartmouth-Hitchcock Health system is seeking a highly skilled Advanced Practice Provider to join our Advanced Heart Failure team at the Heart and Vascular Center.
The ideal candidate will have a strong interest and background in general cardiology and/or advanced heart failure, with a passion for delivering high-quality patient care and education.
As a member of our multidisciplinary team, you will work closely with physicians, APPs, and nurses to provide comprehensive care to heart failure patients and their families.
Responsibilities
- Assess, diagnose, and manage complex and acute cardiac conditions, including cardiomyopathy and heart failure
- Provide education and support to patients and their families, promoting self-management and empowerment
- Collaborate with the multidisciplinary team to develop and implement individualized care plans
- Participate in quality improvement activities and contribute to the development of best practices

Qualifications
- National Certification from an approved certifying body
- Eligible for APRN or PA-C licensure in the State of New Hampshire
- Eligible for Federal DEA registration
