About the Role

We are seeking a highly skilled and compassionate Care Manager Addiction Treatment Specialist to join our team at Brigham and Women's Hospital. As a key member of our OBAT program, you will play a vital role in managing and treating patients with substance use disorders.

Job Summary

The ideal candidate will have a strong background in nursing, experience working with multidisciplinary teams, and a passion for providing person-centered care. You will be responsible for assessing, planning, implementing, and evaluating patient care plans, as well as collaborating with patients and their families to achieve optimal outcomes.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Collaborate with patients, families, and healthcare teams to provide comprehensive care
  • Provide education and support on harm reduction strategies, pharmacotherapy, and other SUD treatment options
  • Conduct regular assessments and reassessments to monitor patient progress and adjust care plans as needed
  • Work closely with the ARP team to ensure seamless transitions of care and coordination of services
Requirements
  • Bachelor's degree in Nursing or related field
  • Current registration in nursing in Massachusetts
  • Certified Addictions Registered Nurse (CARN) or willingness to obtain certification within first year of hiring
  • Minimum 5 years clinical experience, preferably in community health and SUD services
Salary

$74,912 per year, based on qualifications and experience.
