Hospice Social Worker Job Description

Hospice of Holland is seeking a skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will play a critical role in providing compassionate care to patients and their families.

Key Responsibilities:

  • Conduct thorough assessments of patients' and families' needs, identifying areas where support is required.
  • Develop and implement personalized care plans, ensuring patients receive the best possible support throughout their journey.
  • Provide emotional support and counseling to patients and their families, addressing their concerns and fears.
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Document all patient interactions and care activities in a timely and accurate manner.

Requirements:

  • Master's degree in social work from a Council on Social Work Education-accredited program.
  • Current Michigan Social Work licensure as a clinical social worker.
  • Valid Michigan driver's license and reliable transportation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Knowledge of end-of-life care principles and practices, with a commitment to delivering high-quality patient-centered care.
