Job Opportunity: Transitions of Care Clinical Advocate RN
About the RoleThis is an exciting opportunity to join our team as a Transitions of Care Clinical Advocate RN, working closely with patients, families, and healthcare teams to ensure smooth hospital-to-home transitions and advocate for patient needs.
Responsibilities- Collaborate with patients and families to develop personalized care plans
- Coordinate with case management teams to prevent readmissions and ensure seamless transitions of care
- Develop and implement strategies to improve patient outcomes and satisfaction
- Maintain accurate and up-to-date records and documentation
- Communicate effectively with multidisciplinary teams, including physicians, nurses, and social workers
To succeed in this role, you will need:
- A minimum of 2 years' experience in primary care or ambulatory settings with a focus on patient navigation and transitions of care
- An unrestricted RN license in the State of Texas and current Basic Life Support certification
- Experience with Epic electronic health record system and training in Epic end-user programs preferred
$80,000 - $110,000 per year, depending on experience and qualifications.
