Job Summary

We are seeking a skilled and compassionate Home Health LPN to join our team at HCA Midwest Healthcare at Home. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working in collaboration with our multidisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Administer skilled nursing care to patients in their homes, adhering to physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify physical, psychosocial, and environmental needs of patients, documenting findings and communicating with the supervising RN and/or Director of Professional Services.
  • Provide basic and specialized teaching as ordered in the Plan of Care, assessing patient/caregiver knowledge of disease process, treatment regimen, and medication administration.
  • Identify and recommend other disciplines needed for quality care of the patient, collaborating with the healthcare team to ensure comprehensive care.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Valid driver's license.
  • Vocational school graduate.
