Job Summary

We are seeking a skilled and compassionate Home Health LPN to join our team at HCA Midwest Healthcare at Home. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working in collaboration with our multidisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify patient needs and develop individualized care plans.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Provide education and support to patients and caregivers on disease management, medication administration, and other health-related topics.
  • Collaborate with the healthcare team to identify and address patient needs, ensuring timely interventions and optimal patient outcomes.
Requirements
  • Current LPN licensure in the state of employment.
  • Basic Cardiac Life Support certification.
  • 1 year of experience in home health or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
