Job Overview

Position Title: High Fidelity Wraparound Facilitator

Qualifications Required: A Bachelor's degree in a relevant Human Services discipline is essential. Candidates should possess at least one year of experience delivering Targeted Case Management (TCM) services to children and youth with Serious Emotional Disturbance (SED) and their families. A commitment to undergo HFW Facilitator training is necessary. Familiarity with various child-serving systems such as education, behavioral health, child welfare, and juvenile justice is important. Experience in working with families facing complex behavioral health challenges is crucial, along with a strong dedication to empowering these families. Comprehensive knowledge of community resources is expected. Reliable transportation and a valid driver's license are mandatory.

Work Schedule: Standard hours are Monday to Friday from 8:00 AM to 4:30 PM, with potential evening and weekend commitments.

Reporting Structure: This role reports directly to the High Fidelity Wraparound Supervisor.

Key Responsibilities:

  • The High Fidelity Wraparound (HFW) Facilitator ensures that care is provided in a manner that honors family culture, encourages voice and choice, and maintains a strengths-based approach throughout the wraparound process.
  • They provide consultation and education to all service providers regarding the principles of the wraparound process, track progress towards established goals, and ensure that all necessary evaluation data is collected and documented.
  • The HFW Facilitator conducts comprehensive strengths-based assessments of individuals and families, collaborates with team members to create and implement service plans, identifies service providers or natural supports, facilitates team meetings, conducts regular home visits, monitors service delivery, and maintains fiscal accountability.
  • They assist youth and families in developing and integrating their natural support systems and managing their own services and plans.
  • The HFW Facilitator will maintain a caseload of no more than 10 clients.
  • Adherence to documentation deadlines and care plan protocols is essential.
  • They must refrain from engaging in duties unrelated to their primary responsibilities.
  • Timely responses to emergency situations are required.
  • Demonstrating effective collaboration with multidisciplinary team members and the ability to work independently when necessary is crucial.
  • They must follow directions accurately and complete assigned tasks efficiently.
  • Knowledge of issues specific to the population served is expected.
  • Reporting of behaviors or incidents requiring clinical assessment or intervention is necessary.
  • Effective communication of relevant clinical information to other service providers is required.
  • Additional duties may be assigned as needed.

Job Hazards:

  • Work hours may be irregular or flexible based on organizational needs.
  • Routine tasks may involve exposure to blood or infectious materials.
  • All staff must participate in annual training on infection control and safety protocols.
  • New staff must complete training on managing physical and verbal threats, including when police intervention is necessary.
  • Certification in Cardiopulmonary Resuscitation is required within one month of employment.
